Our Program of All-Inclusive Care for the Elderly (PACE), helps participants live independently for as long as possible in their home and within the community. PACE provides added peace of mind for participants and their families with a unique opportunity for you to work with a team of PACE professionals committed to your health and well-being.
PACE has access to a variety of programs to help with your health care needs. From home health to rehabilitation, our PACE team can coordinate a full-range of resources for you including primary and specialty medical care, medications, medical transportation and therapy.
You've spent a lifetime building your home and the wonderful memories within it. Whether you're considering assisted living, a nursing home or senior care, our team at PACE will work together with you and your family to keep you safe and comfortable in your home for as long as possible.
PACE Services
We want you to remain in your home, as active and healthy as possible, for as long as possible. To accomplish that goal, our PACE team will work with you and your family to determine your goals and the right combination of services to meet your needs. Some participants may only require one or two services, while others may require additional services. Our services include, but are not limited to:
- Primary care and specialty medical care (on-site medical clinic)*
- Medications
- Therapeutic recreation
- Transportation to and from UnityPoint Health PACE Senior Care
- Durable medical equipment
- Rehabilitation and restorative therapies
- Dietary services
- In-home support and care
- Social work services
- Respite care
- Hospitalization and emergency care
Note: PACE participant must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from PACE or an entity authorized by PACE. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
Who is Eligible for PACE?
Individuals can join a PACE program if they meet the following conditions:
- Age 55 or older
- Eligible for nursing facility level of care
- Live in a PACE service area county
- Able to live safely in the community at the time of enrollment
What is the Cost?
PACE charges no deductibles or copayments for any drug, service or care approved by the PACE interdisciplinary team.
- If you are eligible for Medicaid and Medicare, you pay no premium for services.
- If you are eligible for Medicaid only, you pay no premium for services.
- If you are eligible for Medicare only, you will be charged a monthly premium for the program, which includes Medicare Part D.
- If you are not eligible for Medicare or Medicaid, our enrollment coordinator will discuss a private pay rate with you.
PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services. Emergency services are covered.
PACE participants may appoint a representative to represent them regarding any claims, appeal, grievance, or request on their behalf by filling out an Appointment of Representative form.
