Our qualified team of experienced home health care professionals work closely with physicians and other health care providers to design and implement a personalized plan of care that addresses the specific needs and goals of every individual. Whether a patient is learning to manage a chronic disease, transitioning home from the hospital, or rehabilitating after an injury or surgery, UnityPoint at Home allows patients to continue the level of care necessary to help restore health, well-being and independence all from the comfort of home.  

Our home care professionals work closely with you, your family and doctor to coordinate your care and manage your health. Your team may include physicians, nurse practitioners, home health aides, medical social workers; physical, occupational and speech therapists and more.

Home Nursing Care

Whether you are a patient managing a chronic disease, transitioning home from the hospital or rehabilitating after an injury or surgery, our qualified care team will work closely with your physicians to create a plan of care that meets your needs and goals. Our home nursing care services can include things like chronic care management, medication management, care coordination and other services. 

Home Infusion Care

Our licensed infusion staff provides a wide variety of nursing care and pharmacy support for IV medications including antibiotics, pain management, chemotherapy, nutritional supplements, hydration and other specialized therapies. Home infusion therapies are prepared under the supervision of specialized pharmacists and are delivered and administered to patients by infusion nurses or trained caregivers. Care can include:
