Our newly designed, climate-controlled mobile unit is customized with two audiogram booths and sound-protective carpeting.

Company Portal/Online scheduling information

PureOHS allows companies to access their results and records quickly through an online portal. To register your company, simply email nicole.wright@unitypoint.org or cailey.hall@unitypoint.org with the following information:

• Name of who needs access
• What records the individual needs access to (Pre-employment or work comp injuries)
• The portal also gives employers access to OSHA logs for injuries. For more information, please review the employer portal guide.

Please note: In an effort to go paperless, we do require an email address be included in the patient demographics in order to send paperwork to be filled out prior to their appointment with our clinic.
