Inpatient Palliative Care, Close to Home
Inpatient Palliative Care collaborates with patients who have been admitted to Finley Hospital. Once a referral is received from your healthcare providers, our team will be in to meet with you and your loved ones to talk about your intentions and wishes.
Our palliative care team will work with your care team to ensure you are receiving the care you want, which aligns with your goals. We will support you as you enter various healthcare settings and assist in communicating your goals to the medical staff to ensure your wishes are respected and honored.
Services at Finley Hospital Palliative Care
- Symptom Management: We can assess and recommend adjustments in the current treatment plan for symptom reduction to augment quality of life, include alternative methods such as Reiki and palliative aromatherapy.
- Advance Care Planning: We can help facilitate the end-of-life decision process, complete Iowa Living Wills, Durable Power of Attorney for Health Care (DPOA), Iowa Physician Scope of Treatment Forms (IPOST), as well as assisting with end-of-life wishes (with Iowa Donor Network).
- Emotional and Spiritual Support: Our clinical chaplains assess cognitive and emotional needs for pre/post-surgery care, and provide:
- A non-judgmental and supportive presence
- Coaching for various relaxation modalities
- Crisis intervention
- Grief counseling/bereavement support
- Facilitation of goals of care meetings with interprofessional team
- Mediation for conflict resolution
- Suicide prevention
- And making referrals to other clinical disciplines.
- Support for caregivers: We can help navigate complex medical and other systems for resources, moral injury support, and support Dementia and other neurodivergent caregivers for spiritual distress and education.
- Community resources and referrals: Communicate and refer to outside clergy or spiritual leaders and resources in the Tri-State area as needed or requested.
