How to Request a Community Sponsorship

We’re proud to support community organizations, projects, activities and initiatives that improve the health, economic and social well-being of the people we serve. As a not-for-profit organization, we believe in the value of giving back to our communities. It's our intention to contribute resources to activities that improve community health and assist community social service organizations, and other non-profit organizations' abilities, to enhance the quality of life in our communities.

Community Sponsorship Request Guidelines

We’ve developed guidelines for requests for community sponsorships, cash contributions and in-kind donations to ensure the support from UnityPoint Health is consistent with our mission to improve the health of the people and communities we serve.

Requirements

The following requests will not be considered:

Annual fund support
Solicitations from for-profit organizations
Political campaigns, candidates, parties or partisan activities
Individuals
Beauty contests
Sectarian, denominational or religious organizations for support of theological functions
Fellowships or internships
Labor or fraternal groups

Community Sponsorship Requests by Location

UnityPoint Health – Des Moines

All organizations seeking a contribution should complete the Community Contribution Request Form. Requests will be reviewed, and applicants will be notified within 30 days via email if the contribution being requested is approved or denied.

Requests will not be considered for programs outside of central Iowa.

UnityPoint Health – Fort Dodge

All organizations seeking a contribution should complete the Community Contribution Request Form. Requests will be reviewed and awarded on a quarterly basis. Notification will be sent by email or mail to all organizations submitting a request for the quarter.

If your request is not granted, it can be rolled over to the next quarter without the need to re-submit the paperwork.

Requests will not be considered for programs outside of the Fort Dodge region.

All organizations seeking a contribution should complete the Community Contribution Request Form. Requests will be reviewed, and applicants will be notified within two to four weeks by email.

Requests will not be considered for programs outside of the Grinnell region.

UnityPoint Health – Quad Cities

To receive information regarding the application process and timeline, please call (563) 742-7610 or email foundation@unitypoint.org. The application window is open from June 1 to July 15 for the following calendar year.

We’ll consider one sponsorship per organization/group per year. If multiple proposals are submitted, we’ll choose the one that best fits with our overall goals. Requests for in-kind and medical sponsorship must also be submitted through the online application process.

Requests will not be considered for programs outside of the Quad Cities region.

All organizations seeking a contribution should complete the Community Contribution Request Form. Requests will be reviewed, and applicants will be notified within 30 days in writing if the contribution being requested is approved or denied.

Requests will not be considered for programs outside of the Sioux City region.

UnityPoint Health – Waterloo

All organizations seeking a contribution should complete the Community Contribution Request Form. Requests for sponsorships will be reviewed and applicants will be notified within 45 days of receipt via email and/or in writing whether the contribution being requested is approved or denied.

One sponsorship per organization/group will be considered per calendar year for entities located in the Cedar Valley. Requests for in-kind must also be submitted through the online application process.
