Qualifications
Requirements

Candidates must possess a Doctorate of Pharmacy degree or equivalent experience from school of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) and be eligible for licensure to practice pharmacy in their respective state. Candidates must also participate in the ASHP Resident Matching Program.

As an ASHP-Accredited site, we participate in the ASHP residency matching process. Candidates must complete the Phorcas residency application before the deadline listed on the ASHP residency site. It must also include a personal statement/letter of intent, curriculum vitae and verified pharmacy education transcripts.

Resident staffing is every third weekend, two holidays, two evenings every three weeks, two shifts during the week between Christmas day and New Year's day 0700-1530 and two shifts during local spring break week (date TBD yearly) 0700-1530.

Learning Experiences

The residents participating in our 12-month residency will be accountable for optimizing drug therapy and achieving the best outcome for every patient, every time as a member of the inpatient healthcare team. The residents will provide drug information and education to patients, other healthcare professionals, and students. Our residency program is designed to develop practitioner excellence in the provision of patient-specific, evidence-based care and will focus on the development of knowledge and skills needed to become a competent and confident practitioner. Graduates of this program will be prepared to:

About Allen Hospital

The hospital features an electronic medical record and bedside barcode scanning with computerized physician order entry. The Department of Pharmacy has extensive automation, technology, and a state-of-the-art IV clean room. The pharmacy provides 24-hour service to inpatients at Allen and to four additional affiliates, and third-shift service for multiple affiliates across the state. In addition, Allen offers outpatient services through a retail pharmacy, two anticoagulation clinics, community cancer center and an ambulatory surgery center support pharmacy. The pharmacy acts as a training site for the experiential education of introductory and advanced practice experience pharmacy students from University of Iowa College of Pharmacy and advanced practice experience students from Creighton University College of Pharmacy.

Allen Hospital PGY-1 Residency

Julia Fifield

PharmD (Residency Program Director)

Julia joined Allen Hospital as a clinical pharmacist in 2018. She is originally from Naperville, IL. Julia received her Doctor of Pharmacy degree from the University of Iowa in 2017 and completed her PGY1 Pharmacy Residency at Allen. She is adjunct faculty at the University of Iowa and precepts APPE students on their general hospital rotation. Julia serves as RPD, preceptor for infectious diseases, and the co-chair of antimicrobial stewardship. In her free time, she enjoys reading, listening to Taylor Swift, baking sourdough bread and spending time with her husband and kids.

Amy Kacher

PharmD, (Residency Coordinator)

Amy joined Allen Hospital as a full-time staff/clinical pharmacist in 2001. She graduated from the University of Iowa College of Pharmacy with a Doctor of Pharmacy degree in 2000 and completed a pharmacy residency with emphasis in family medicine at Northeast Iowa Family Practice Center in Waterloo, Iowa in 2001. She is an Adjunct Assistant Professor for the University of Iowa College of Pharmacy, precepting an Acute Care rotation for APPE students. Professional interests include cardiology, anticoagulation, emergency medicine, and critical care.

Zach Lynch

PharmD (Distributive Pharmacy Manager)

Zach joined Allen Hospital as a full-time clinical pharmacist in 2020. He graduated from the University of Iowa College of Pharmacy in 2019 and completed his PGY-1 Pharmacy Residency at Allen Hospital from 2019-2020. Zach is the distributive pharmacy manager and is also a co-preceptor for the infectious disease learning experience for the residency. His professional interests include infectious disease, critical care and diabetes. In his free time, Zach enjoys golfing and spending time with his family on their farm.

Jacob Peterson

PharmD (Clinical Program Manager)

Jacob received his Doctor of Pharmacy degree from the University of Iowa in 2016. He completed his PGY1 pharmacy residency at Allen Hospital. He has worked as a clinical pharmacist both at Allen Hospital and in Colorado and has most recently taken the role of Pharmacy Clinical Manager. He precepts residents on the Longitudinal Practice Management experience. His professional interests include infectious diseases and critical care. In his free time, Jake enjoys traveling and spending time with his wife and three daughters.

Tim Schneider

PharmD (Regional Pharmacy Director)

Tim has been with UnityPoint Health – Waterloo since 2011. He earned a Doctor of Pharmacy degree at the University of Iowa in 2005 following undergraduate education at the University of Northern Iowa. He currently serves as the multiregional director of pharmacy services. When he is not at work you can most likely find him hanging out with his wife, Leah, and their three kids. Tim and Leah met in pharmacy school and she is currently a pharmacist at BCHC Oelwein Clinic Pharmacy. Their three children attend Wapsie Valley Schools and enjoy all things involving sports and the outdoors.

Mackenzie Monroe

PharmD, BCPS, BCCCP

Mackenzie joined Allen Hospital in October 2019. She graduated from Iowa State University in 2007 with a Bachelor of Science degree in agricultural biochemistry and a minor in genetics. She continued her education at Belmont University in Nashville, TN where she received her Doctor of Pharmacy degree in 2012. Mackenzie went on to complete her PGY1 Pharmacy Practice Residency at MercyOne Des Moines Medical Center. Following residency, she continued working at MercyOne in critical care, the emergency department, and pediatrics before coming to Allen. Her professional interests include critical care, emergency medicine, and infectious diseases.

Susan Mueller

PharmD, BCPS

Susan joined Allen Hospital in 2022. She studied biochemistry while working as a research assistant in pharmacology at the University of Iowa. Susan received a Doctor of Pharmacy degree in 2017 and completed the PGY1 pharmacy practice residency at Covenant Medical Center. Susan is one of the preceptors for Emergency Medicine.

Beth Nichols

BA, PharmD

Beth has been with Allen Hospital since 2005. She graduated from the University of Northern Iowa in 2001 with a BA in Biology and a minor in Chemistry. She went on to the University of Iowa where she received her PharmD in 2005. She is an adjunct faculty member at the University of Iowa where she precepts General Hospital APPE students. She is also the preceptor for the longitudinal pain learning experience and the Palliative Care/Hospice learning elective for the PGY1 residency. Professional interests include pain management, hospice, palliative care and geriatrics.

Linsey Schuldt

PharmD, BCGP

Lindsey joined Allen Hospital in 2004. She graduated from University of Iowa College of Pharmacy with her PharmD in 2003 and completed her pharmacy residency with an emphasis in family medicine at Northeast Iowa Family Practice Center in Waterloo, IA the following year. In 2017, she achieved board accreditation in geriatrics and offers a geriatrics elective with the residency program. She is an Adjunct Assistant Professor for University of Iowa College of Pharmacy, precepting an Acute Care rotation for APPE students. Linsey serves on the Falls Committee and Patient Safety Committee in the hospital, and on the Palliative Care team. Her pharmacotherapeutic interests include palliative care, long-term care, and geriatrics.

Angie Slickers

PharmD

Angie joined Allen Hospital in 1999 and has worked in the retail, inpatient and ambulatory settings. She graduated from the University of Iowa College of Pharmacy. Angie currently precepts the anticoagulation learning experience. Her professional interests include anticoagulation and geriatrics. In her free time, Angie enjoys spending time with her family and visiting the Cedar Falls Recreation Center.

Lisa Veit

PharmD, BCPS

Lisa joined UnityPoint Health in 2007 and transitioned from Allen Hospital to Grundy County Memorial Hospital in 2023. She has been a relief pharmacist at Buchanan County Health Center since 2008. She graduated from University of Iowa College of Pharmacy with her PharmD in 2005. She then completed her pharmacy practice residency at Alegent Health – Bergan Mercy Medical Center in Omaha, NE the following year. Her pharmacotherapeutic interests include emergency medicine, infectious disease and rural care. Lisa enjoys spending time with her husband, four children and their dog, in addition to reading, crafting and cheering on the Hawkeyes.

Andrew Washburn

PharmD

Andrew joined Allen Hospital as a full-time anticoagulation clinical pharmacist in July 2013. He graduated from the University of Iowa College of Pharmacy with a Doctor of Pharmacy degree in 2012. He completed a PGY1 Pharmacy Residency at Allen Hospital from 2012-2013. He precepts APPE and IPPE students and enjoys teaching pharmacy students, providers, and nurses. Professional interests include anticoagulation, cardiology, infectious disease, emergency and critical care medicine. He anticipates teaching classes at a college of pharmacy. He also plans to create an APPE learning experience with focus in ambulatory clinical care in the future.

PGY-1 Resident - Allen Hospital Program - Bradley Johnson

Bradley Johnson, PharmD, is originally from Racine, Wisconsin. He recently received his Doctor of Pharmacy from the University of Minnesota on the Duluth campus. His professional interests include pharmacokinetics and medication management. Upon completion of his PGY-1 Pharmacy Residency, he plans to pursue pharmacy leadership to improve pharmacy access and address system-based problems. In his free time, he can be found outside hiking and exploring nature with his camera and drone. He also enjoys experiencing different cultures through food.

PGY-1 Resident - Allen Hospital Program - Christopher Le

Christopher Le, PharmD, is a first-generation Vietnamese American and a native Chicagoan. He received his doctoral pharmacy degree from the University of Illinois at Chicago and Masters degree at Northwestern University. His professional interests revolve around direct patient-care including emergency medicine, critical care, and pain management. Outside of pharmacy, he enjoys playing cello and piano; he also loves to participate in city running events such as the Bank of America series and the Hot Chocolate Run.

PGY-1 Resident - Allen Hospital Program - Kindra Welter

Kindra Welter, PharmD, is originally from Gilbertville, Iowa. She attended the University of Iowa College of Pharmacy where she received her Doctor of Pharmacy degree in May 2024. Her current professional interests include critical care, oncology, and infectious disease. Kindra hopes to obtain a clinical pharmacist position in the area following her residency.

Elizabeth Baumgarn, PharmD is originally from central Iowa. She earned her Doctor of Pharmacy degree from the University of Texas at Tyler Ben & Maytee Fisch College of Pharmacy. Her professional interests include geriatrics and transitions of care. Upon completion of her PGY-1 Pharmacy Residency, she plans to pursue board certification and provide patient care in an academic environment. In her spare time, she can be found outside with her dog or baking sweet treats.

Her current professional interests include psychiatry, transitions of care and emergency medicine. Following her PGY1, she hopes to obtain a PGY2. In her free time, she enjoys spending time outdoors, reading and enjoying live theater.

Madeline Zettel, PharmD is originally from Menasha, Wisconsin. She recently received her Doctor of Pharmacy degree from Midwestern University in Glendale, Arizona. Her professional interests include emergency medicine, cardiology and surgery. Following residency, Mattie plans to pursue a board certification and look for a clinical position in a community hospital. In her free time, she enjoys camping, riding horses, playing soccer or catching a sports game. Go Pack Go!

AJ Dollmeyer, PharmD is originally from Polo, Illinois. He attended Augustana College in the Quad Cities where he received a Bachelor of Science degree in biochemistry. He recently received his Doctor of Pharmacy degree from the University of Iowa in May 2022. His current professional interests include infectious disease, cardiology and critical care. Following residency, AJ is contemplating a PGY2 residency next year and long-term plans to obtain a clinical pharmacist position at a community hospital.

Kassidy Lawler, PharmD is originally from Burlington, Iowa. She received her Doctor of Pharmacy degree from the University of Iowa in May 2022. Her current professional interests include infectious disease and emergency medicine. Kassidy hopes to obtain a clinical pharmacist position in the area following her residency. Go Hawks!

Logan Mougin, PharmD is originally from Bettendorf, Iowa. He attended the University of Iowa where he received his Doctor of Pharmacy degree in 2021. Prior to residency he worked in the Chicago area in a community/compounding pharmacy. His current professional interests include critical care, infectious disease and pain management. He hopes to obtain a clinical pharmacist position in Iowa or Illinois

Crystal McElhose, PharmD is originally from Hinton, Iowa, near Sioux City. She received her Doctor of Pharmacy degree from the University of Iowa in May 2021. Her current professional interests include cardiology, infectious disease, internal medicine and anticoagulation management. She is looking forward to providing confident and compassionate care to her patients, while building upon her leadership skills and clinical knowledge and judgement. Following residency, Crystal accepted a position with UIHC Children's Hospital.

Clayton Miller, PharmD is originally from West Salem, Wisconsin. He received his Doctor of Pharmacy degree from the University of Colorado in May 2021. His current professional interests include critical care, infectious diseases and pharmacogenomics. Clayton is moving back to Colorado following residency and looking for a clinical position in the Denver area.

Ashraf (Ash) Amadou, PharmD is originally from Benin in West Africa. He attended Bloomfield College in New Jersey where he received a minor in mathematics and a Bachelor of Science degree in chemistry and biology. He recently completed his Doctor of Pharmacy degree from South Dakota State University. His current professional interests include cardiology, infectious diseases, hospital administration, critical care, and pain management. Following residency, Ash plans to pursue a board certification and is contemplating a PGY2 residency.

Hannah Hemry, PharmD is originally from West Union, Iowa. She attended the University of Northern Iowa in Cedar Falls, IA where she received a Bachelor of Science degree in biochemistry. She received her Doctor of Pharmacy degree from the University of Iowa in May 2020. Her current professional interests include cardiology, internal medicine and critical care. Following her residency, Hannah took a job with UIHC in Iowa City.

Karli Welter, PharmD is originally from Gilbertville, Iowa. She received her Doctor of Pharmacy degree from the University of Iowa in May 2020. Her current professional interests include cardiology, oncology, and critical care. Karli accepted a job with MercyOne hospital in Waterloo.

Payton Augustine

PharmD

Payton joined UnityPoint Health – Prairie Parkway Pharmacy as a medication therapy management pharmacist in 2022. She graduated from the University of Iowa College of Pharmacy with her PharmD in 2021. Prior to becoming an MTM pharmacist she completed a PGY-1 Community Pharmacy Residency with UnityPoint from 2021-2022. Her professional interests include chronic disease state medication management and continual expansion of the role of pharmacy as a clinical pharmacist.

Michael Boller

PharmD

Michael joined UnityPoint Health – Prairie Parkway Pharmacy as clinical pharmacist in 2021. He graduated from the University of Iowa College of Pharmacy with a PharmD in 2020 and completed PGY1 Community Pharmacy Residency in 2020-2021. He primarily practices at UnityPoint Clinic Internal Medicine – North Crossing Clinic in Waterloo and his interests include related chronic disease states including anticoagulation, diabetes and COPD management.

Amy Bucknell

PharmD

Amy joined Allen Hospital in 1999. She graduated from the University Of Iowa College Of Pharmacy and has served as a Retail Pharmacy Manager in the Waterloo region since 2006. She credits her outstanding retail teams with the growth and development of programs like Med-to-Bed through Allen Clinic Pharmacy, SNF discharge through Prairie Parkway Pharmacy, Discharge Med Rec, MedSafe Drug Disposal, Immunization and Med Packaging Services and many others. She currently takes a lead role in the Prairie Parkway Care Coordination Committee and is active in the UPH system OPOT (UPH outpatient pharmacy operations) team. She enjoys learning from our community residents, IPPE and APPE students at both Allen Clinic and Prairie Parkway Pharmacies. Her professional interests include retail pharmacy process improvement and sustainability.

Chelsea Carlson

PharmD (Residency Coordinator)

Chelsea has worked for UnityPoint Health at Prairie Parkway Pharmacy since the building first opened in 2016. She is a graduate of the University of Iowa College of Pharmacy and serves as the Lead Pharmacist and Residency Coordinator at Prairie Parkway. Chelsea enjoys the unique experience Prairie Parkway offers in which she has the retail services she enjoys as well as time spent in the family medicine clinic and site management duties. She is a continuous learner and likes the experience that being an IPPE, APPE and residency preceptor has to offer. She hopes to be part of the continued growth the pharmacy and residency programs offer. When not working, she enjoys spending time with her family, including three sons, and cheering on the Iowa Hawkeyes.

Lisa Duric

PharmD

Lisa graduated from the University of Iowa College of Pharmacy in 2014 and started with UnityPoint Health – Waterloo in 2016. She worked with the inpatient pharmacy, then when the opportunity arose to join the newly created Prairie Parkway Pharmacy and clinics she took it. Lisa enjoys the combination of retail and clinic pharmacy she experiences at Prairie Parkway. Her professional interests include inventory management and mental health.

Nichole Nunemaker

PharmD

Nichole joined UnityPoint Health in 2020 as a PGY-1 resident after attending the University of Iowa College of Pharmacy. After residency she was hired as both an MTM pharmacist and retail pharmacist, splitting her time between each role. As of July 2022, she is now a full-time MTM pharmacist working in Allen Hospital's new 340b clinic helping to manage diabetes, COPD/asthma, anticoagulation, as well as long-acting injectable antipsychotic medications. Her professional interests include the disease states she manages through the 340b clinic as well as assisting patients with lifestyle modifications and providing lower income patients with better access to their medications.

Nicole Sivertsen

PharmD

Nicole joined UnityPoint Health in December 2016. She graduated from the University of Iowa in 2015. Nicole currently serves as Pharmacist Team Lead at Allen Clinic Pharmacy and an adjunct faculty member at the University of Iowa where she precepts Community Pharmacy APPE students. Her pharmacotherapeutic interests include chronic disease management and immunizations.

Amanda Waggett

PharmD, CDE

Amanda joined UnityPoint Health - Prairie Parkway Pharmacy as a clinical pharmacist in 2018. She graduated from University of Iowa College of Pharmacy with her PharmD in 2017 and completed her PGY1 Community Pharmacy Residency with UnityPoint Health from 2017-2018. She received her Certified Diabetes Educator certificate in 2020. Her professional interests include diabetes, anticoagulation, and chronic disease state management.

Christine Chang, PharmD is originally from Chicago, Illinois. She attended the University of Illinois at Chicago, where she received her Bachelor of Arts degree in psychology in 2018 and her Doctor of Pharmacy degree in May 2022. Her current professional interests include diabetes, asthma/COPD and chronic disease state management. She hopes to obtain a position as an ambulatory care pharmacist in Illinois or Iowa.

Kalyca Nardy, PharmD is originally from Cedar Rapids, Iowa. She attended Iowa State University where she graduated with a Bachelor of Science degree in Genetics. She recently received her Doctor of Pharmacy degree from University of Iowa in May 2022. Her current professional interests include cardiology, diabetes and anticoagulation management. Frequently Asked Questions

Are all residents from Iowa or Iowa pharmacy schools?

No, Allen Hospital considers all residency applicants and strives to obtain diversity in the educational backgrounds of the residents. However, we are not able to sponsor Visas.

Do I need to have an Iowa license?

YES! All residents are required to pursue licensure in Iowa prior to beginning the residency training program and complete by August 1. If a resident fails to be licensed by August 1, the residency contract will be terminated.

Do residents have Internet and email access

Each resident is assigned a computer and will have access to computers in each nursing station as well as the pharmacy. All computers have internet access and email capabilities. Free Wi-Fi is available onsite.

Do residents have library privileges?

Residents have access to Allen College's library as well as online references from the Hardin Library of Health Sciences – University of Iowa. The Micromedex, Pharmacist's Letter and DynaMed systems are available through the hospital's intranet.

Do residents receive support to attend professional meetings?

Financial support is provided for all required meetings (ASHP MCM, Midwest Pharmacy Residency Conference). Total support includes hotel accommodations, airfare, conference registration, and meals to a maximum of $1000 unless otherwise specified.

How are residents evaluated?

At the beginning of the residency year, the resident will complete an initial assessment and the residency director will use this to create a development plan. Flexibility has been built into the program to allow the resident to adapt the program to meet their interests and focus on identified areas for improvement. This plan will be updated quarterly. The resident must be evaluated at the end of each learning experience by his/her preceptor. How are the residents selected?

Resident selection involves an application process through PhORCAS (Pharmacy Online Residency Centralized Application) and an onsite interview. Interviews are held in January and February of each year.

How is the resident's year structured?

Allen Hospital PGY-1 Program: The first 6-8 weeks of the residency involves orientation. During this period, the resident will spend time training in various pharmacy and patient care areas learning the pharmacy and hospital computer systems, becoming BLS and ACLS certified (if not already), exploring potential research projects, etc. Clinical rotations are typically 4-8 weeks. A research month may be scheduled based upon the magnitude and number of extra projects taken on by the resident in addition to appropriate progression of clinical experience. A week in December is reserved for attendance at the ASHP Midyear Clinical Meeting.

Community-Based PGY-1 Program: The first 4-6 weeks of the residency involves orientation. During this period, the resident will spend time training in various pharmacy and patient care areas learning the pharmacy and computer systems, becoming BLS certified (if not already), and exploring potential research projects, etc. Once orientation is completed, the resident will provide clinical services for the pharmacy and adjoining clinics. This may include but not be limited to MTM, disease state management, vaccine services (on and off site), discharge medication reconciliation and long-term care management. As a new residency, the potential for additional services is open. Residents may also have opportunities for experiences in the hospital.

How much is the stipend?

The PGY-1 stipend is approximately $48,505. Residents are also offered a benefits package – more details provided upon request.

Is a transcript required?

Yes. Although the most current transcript may not be necessary. A transcript missing one or two clerkship/clinical rotation grades is easily supplemented by any letters or recommendations that are written by the preceptor/s of those rotations. In addition, a complete transcript can be sent at any time prior to January 30, once the final fall semester/quarter grades are available provided that a recent transcript has already been sent.

What else is there to do in the area?

Waterloo and Cedar Falls have an abundance of recreational opportunities including museums, visual performing arts venues, sports teams and arenas, and leisure activities. There is also an extensive bike and walking trail system throughout the Cedar Valley. A variety of local multi-cultural activities exist – see links below for calendars of events in the area. Both communities are family-friendly environments welcoming a diverse array of people from all walks of life.

Waterloo's shopping mall and movie theater are just the beginning of what the town offers. The outdoor water park accommodates hundreds of people daily and is a great family-friendly attraction. Waterloo is also home to a casino as well as two sports teams. The Waterloo Bucks are a pleasure for local baseball fans and the Waterloo Black Hawks hockey team floods Young Arena with its fans each season. Waterloo is home to Hawkeye Community College, Allen College, and an off-site alternative location for many other small colleges and universities.

Cedar Falls has many artistic venues to enjoy and a Main Street District that is enjoyable for all ages. With its historically renovated shops, theaters and art studios, as well as great restaurants, the area offers activity options for everyone. Cedar Falls is also home to the University of Northern Iowa. Not only does this allow for Panther sporting events and a beautiful campus to enjoy, but a well-rounded calendar of events for all interests.

The rural Cedar Valley also has hidden treasures not to be missed. Small town shops, town fairs and festivals, and beautiful scenery all give you the opportunity to engage in the rural life of local Iowans. There are plenty of things to see with just a short distance to drive. However, the Cedar Valley is conveniently located in the midst of several major cities that have their own flavor and attractions.

What is involved in the staff requirement?

Allen Hospital PGY-1 residents will provide weekend pharmacy coverage rotating every third weekend (one resident per weekend), generally an eight-hour shift (0700-1530), also two evening shifts per three-week cycle (1500-2130) and two assigned holidays (0700-1530). Community-Based PGY1 residents will provide weekend pharmacy coverage. Your schedule will be provided at the beginning of the program.

What is the average cost for housing in the Waterloo-Cedar Falls area?

You should expect to pay $400 – $1200/month for a one- or two-bedroom apartment. Housing will need to be arranged by the resident. Contact the Residency Coordinator or other pharmacy staff for suggestions on the area if needed.

What teaching opportunities are available?

Pharmacy residents will participate in the University of Iowa College of Pharmacy's Pharmacy Educator Preparation Program for Residents (PEPPR) teaching certificate program. Allen Hospital (AH) maintains an affiliation with Allen College of Nursing and University of Iowa College of Pharmacy as a clinical teaching site for students. AH residents have opportunities to assist in the teaching of students from multiple disciplines including pharmacy, medicine, nursing, and nurse practitioners. Opportunities for didactic teaching may be available within the College of Pharmacy or Allen College of Nursing.

When is the application deadline?

The application, curriculum vitae, college transcripts, three letters of recommendation and a letter of intent must all be submitted by January 6, 2021 via the PhORCAS system. Interviews are planned to begin mid-January and throughout February.

When is the start date?

Typically, the Monday of the second to last or last week of June, with July 1 as latest possible start date. Start date is based on pay periods for the year. The start date will be communicated to the matched residents in April.

Will I need to work holidays?

Yes, each resident will work at least two and a maximum of three holidays throughout the year. These will be assigned. If a resident cannot work on a scheduled holiday, they will need to negotiate a trade with the other resident.
